About 2017 Model Practices
NACCHO’s Model Practices Program nationally honors and recognizes outstanding local public health practice and shares the outstanding practices through the Model Practices Database. By submitting a practice, LHDs contribute to the overall improvement of public health through effective evidence-based practice methods.
NACCHO strives to recognize the best local public health practices in the country. This is why we created the Model Practices Awards.

Eligibility Details
Eligibility
All local health departments (LHDs) and NACCHO affiliate members are eligible to submit practices; however, only active (dues-paying) NACCHO members will be eligible to receive a stipend to attend the NACCHO Annual Conference (pending available funding for stipend).
Deadlines
To be eligible for the 2016 Model Practices Awards, online submissions must be received by 11:59 PM EST on December 9, 2016. We strongly encourage early submissions.
